FOX 47’S TOUCHING LIVES NURSE OF THE MONTH

Who Can Enter
1. To be eligible to participate in this contest, entrants must be U.S. or Wisconsin residents who are 18 years of age or older and reside in the WMSN viewing area.
2. Employees of Sinclair Broadcast Group, Inc., Sinclair Television Group, Inc., Sinclair Television Company Inc., and any other company which owns programs or provides other services to television broadcast station WMSN (the “Station”), S.E.I.U. - Service Employment International Union, their affiliated companies, advertising and promotion agencies, any and all local television broadcast and cable stations, local radio and other media companies, and the immediate family members of each are ineligible. The term “immediate families” includes spouses, grandparents, parents, siblings, children and grandchildren.
3. Nominator cannot be an immediate family member to the nominee. The term “immediate family” includes spouses, grandparents, parents, siblings, children and grandchildren. Patients may not nominate a nurse or nurse educator with whom they have a current relationship.
How to Enter
(a) No purchase is necessary to enter. Only one entry per contestant.
(b) To nominate a Wisconsin licensed registered nurse (RN) or licensed practical nurse (LPN) go to www.fox47.com, fill out the entry form and release. Official entry forms and official rules may be picked up at Fox 47 7847 Big Sky Drive Madison WI 53719 during business hours Monday through Friday 8am to 5pm, September 3, 2007 through August 31, 2008. No mechanically reproduced entries will be accepted. Entry form and releases may be scanned with signature and emailed if you are over the age of 18. The Station is not responsible for lost, late, or misdirected mail or for printing errors, server unavailability, computer or any other electronic malfunction.
(c) The contest starts on September 3, 2007 and all entries must be received by 5p.m. on August 31, 2008 (the “Contest Period”). Each entry must have a written or typed essay of 500 words or less describing why a RN or LPN should be featured on Fox 47’s Touching Lives campaign. All entries must be nominator's original work and should not be/have been submitted to or published by any other publication. All nominations must also contain valid contact information for two (2) professional references. Entrants must disclose the professional basis of the relationships between the nominator, the nominee and the two (2) references. The professional references provided must be from licensed nursing professionals, physicians, physician's assistants, or nursing or medical faculty members.
(d) One (1) winner and one (1) alternate winner per month during the Contest Period will be selected by a panel of four (4) judges on the last business Wednesday of each month beginning September 2007 and ending August 2008 based on the following criteria: legibility of essay 25%, evidence of Nurse’s compassion 25%, demonstration of Nurse’s medical knowledge 25%, unique examples of Nurse’s attentiveness 25%. All judges decisions are final.
(e) All entries received that are mutilated, tampered with, illegible, or from ineligible entrants will be void. All entries become the property of Sinclair Broadcast Group, Inc., Sinclair Television Group, Inc., and Sinclair Television Company Inc., SEIU - Service Employment International Union, and will not be acknowledged or returned.
The Prize(s)
1. Twelve (12) Touching Lives winners will be selected, one per month, for one year, and each will receive one (1) getaway for two (2) at Grand Geneva Resort & Spa. The prize package includes two (2) consecutive nights hotel accommodations (one room, double occupancy) at Grand Geneva Resort & Spa, Sunday through Thursday excluding holidays. The package does not include any meals, transportation, gratuities, trip cancellation insurance, or any other expenses. The prize must be used by one (1) year from date of issue, is subject to availability, and blackout dates may apply. The trip package is valued at $885. Winners also receive a recognition bowl valued at $87.50. Total prize package valued at $972.50. The Winners will be interviewed on Fox 47 News at 9 for the Touching Lives news segments.
2. Winners will be notified by phone. Winners must sign and return a release and affidavit of eligibility, which must be received within three (3) days of notification. Winners must show picture identification to claim prizes. The Station reserves the right to examine additional identification and may choose to accept or deny awarding the prize based on the identification presented.
3. Winners will be subject to a background check, reference check, and must agree to be interviewed and videotaped, along with their nominators, references and patients, by the Station for news and promotional purposes. Winners will be disqualified if the background check of public criminal records reveals felony charges or convictions. Winners will be disqualified if professional reference checks can not be verified within 3 (three) business days.
4. Failure of the winner to collect the prize within three (3) days will constitute forfeiture of the prize. If forfeited, the alternate winner will be contacted. The alternate winner will have three (3) days to collect the prize and sign affidavit of eligibility and release, and is subject to a background and reference check.
5. There will be no substitutions or cash alternatives for prizes except at the sole discretion of the Station. The Station has the right to substitute a prize of similar value. No transfer or assignment of prizes is allowed.
6. The winner is responsible for paying all local, county, state and federal taxes on prizes based on the estimated retail value of the prizes as set forth in these rules.
7. Only one prize will be awarded per household.
8. Sinclair Broadcast Group, Inc., Sinclair Television Group, Inc., and Sinclair Television Company Inc., are not liable if the provider of any part of the prize fails to satisfy its obligations to furnish its portion of the prize. The Station, sponsors, and prize provider shall not be responsible for winner’s failure to use the prize within the time frame described above, or cancellation of the trip by the winner
9. Sinclair Broadcast Group, Inc., Sinclair Television Group, Inc., and Sinclair Television Company Inc., are not responsible for any liabilities arising directly or indirectly from the award or use of the prize.
10. In claiming your prize, you acknowledge that Sinclair Broadcast Group, Inc., Sinclair Television Group, Inc., and Sinclair Television Company Inc., has the right to publicize your name, character, likeness, photograph, voice and the fact that you are the winner for promotional purposes without financial remuneration.

1. Odds of winning depend on the number of entries received.

3. For a list of prize winners, send a separate, self-addressed, stamped envelope to Fox 47’s Touching Lives Awards 7847 Big Sky Drive Madison, WI 53719 by September 31, 2008.
4. This contest is subject to all applicable laws and regulations and is void where prohibited. Sinclair Broadcast Group, Inc., Sinclair Television Group, Inc., Sinclair Television Company Inc., S.E.I.U. - Service Employment International Union, accept no responsibility or liability in connection with any injuries, losses or damages of any kind caused by or resulting from the acceptance, possession or use of any prize awarded hereunder.
5. Sinclair Broadcast Group, Inc., Sinclair Television Group, Inc., and Sinclair Television Company Inc., cannot be held liable for any lost or stolen entries, prizes or contest information.
6. Sinclair Broadcast Group, Inc., Sinclair Television Group, Inc., and Sinclair Television Company Inc., reserves the right to make rules and contest changes and change the aforementioned contest dates.
