   FOX 47 Touching Lives Award for Nurses

Do you know a nurse who has made a difference for you, a friend or a family member? Did you have an experience with a nurse who displayed extensive medical knowledge while being unusually attentive and compassionate? Then, nominate them for their work and dedication to patient care and let our community learn of their achievements! This is your chance to return the kindness that they share with their patients everyday.

Nominate a special nurse in an essay of 500 words or less that describes your unique experience. Share your story and help shine the spotlight on the nurse that you believe is “Touching Lives”.

Then, each month Service Employment International Union 1199 Wisconsin and Fox 47, will select a nurse from viewer nominations. Fox 47 News at 9 will feature the selected nurse during a “Touching Lives” segment the second Sunday of each month (and here on fox47.com) beginning October 14th, 2007.

You must be 18 to enter.

The nurse you nominate must be a Wisconsin licensed registered nurse (RN) or licensed practical nurse (LPN).

You can not nominate your own immediate family member who is a nurse.

You must provide two (2) professional references for the Nurse with name, phone, address. The professional references provided must be from licensed nursing professionals, physicians, physician's assistants, or nursing or medical faculty members.

How to enter by email if you are over the age of 18:
(remember you may not be related to the nurse you are nominating)

  • Download and print the entry form and release from above
  • Fill out your information and sign the release by hand.
  • Scan the 2 page entry form and release with your signature and information
  • Attach the 2 page entry form and release with your signature using a
    a .pdf or .jpg format (no Word documents, or any Mac format accepted)
  • Write your essay in the body of the email
  • Send the email to contests@fox47.com (click the link or send with a subject line of "WMSN Nurse Nomination")

Top Promotions

Winner must use the prize one year from issue date, prize does not include transportation, meals, parking, or any other expenses. Winners pay all federal, local, state, income taxes based on value of prize stated above.

Winners will be subject to a background check, reference check, and must agree to be interviewed and videotaped, along with their nominators, references and patients, by the Station for news and promotional purposes. Winners will be disqualified if the background check of public criminal records reveals felony charges or convictions. Winners will be disqualified if professional reference checks can not be verified within 3 (three) business days.

One (1) winner will be selected by a panel of four (4) judges on the 1st business Monday of each month beginning October 1, 2007 and ending October 1, 2008 based on the following criteria: legibility of essay, evidence of Nurse’s compassion, demonstration of Nurse’s medical knowledge, unique examples of Nurse’s attentiveness. All judges decisions are final. Entries will not be acknowledged or returned.
